Prematürite / Prematurity

2 Mayıs 2012 tarihinde dünyada ilk defa ulusal, bölgesel ve global erken doğum oranları raporu yayınlandı. Çok Erken Doğan bebeklerin ülkeler bazında zamanında doğanlar ile sayısal karşılaştırması, bu oranlar bazında ülkelerin sıralaması, Çok Erken Doğan bebeklerin ülke bazında yaşama oranları ile bu oranlar bazında ülkelerin sıralaması raporda yer alıyor.

15 milyon bebeğin erken doğduğu ve her yıl 1 milyondan fazlasının öldüğünün bildirildiği raporda erken doğum oranlarının birçok ülkede artış gösterdiği ve erken doğumun zatürreden sonra beş yaşın altındaki bebek ölümlerindeki ikinci en yüksek orana sahip sebep olduğu kaydediliyor.

3 Mayıs 2012 tarihinde ise World Health Organization - WHO (Dünya Sağlık Örgütü) ve The Partnership for Maternal, Newborn and Child Health - PMNCH (Gebelik, Yenidoğan ve Çocuk Sağlığı Ortaklığı) tarafından organize edilen 7 saatlik twitter paylaşımı konuyla ilgili uzmanların katılımı ile düzenlendi.


On 2 May 2012 The Global Action Report has been released on Preterm Birth which provides the first-ever national, regional and global estimates of preterm birth. The figures comparing the number of term babies with the ones Born Too Soon are given on country basis and including a ranking as well as the rates of survival on country basis and their ranking.

It is reported that, 15 million babies are Born Too Soon and over 1 million die each year. The report shows the extent to which preterm birth is on the rise in most countries, and is now the second leading cause of death globally for children under five, after pneumonia.

ROP / Retinopathy of Prematurity

Birçok erken doğanın korkulu rüyalarından biri, ROP...

Doğum haftası erkenleştikçe ve doğum ağırlığı düştükçe riskte artıyor. "Zone"u var, "evre"si var. Teşhis ve tedavisi mümkün ama kritik kontrol noktalarını kaçırmamak önemli!

Özetle, süreç şöyle işliyor:
- doğum ile beraber retinadaki damarlanma duruyor
- erken doğum durumunda, damarlanma tamamlanamadan durmuş oluyor
- anne karnındakinden farklı olarak fazla veya eksik oksijen alımına bağlı olduğu düşünülen, retinada anormal bir damarlanma başlıyor
- bölgesi (zone I-II-III) ve düzeyi (evre 1-2-3-4-5) ROP konusunda deneyimli göz doktorları tarafından yapılan periyodik kontrollerde değerlendiriliyor
- evre 1-2 izlemeye devam edilmesi gerektiğini belirtiyor
- evre 3 müdehale gereksinimini ortaya koyuyor, genelde laser yapılıyor
- evre 4-5 müdehalenin zorlaştığı, görme kaybının gündeme geldiği süreç...

Ben bugün bildiklerimi o gün biliyor olsaydım ve erken doğum olasılığını da dikkate alarak doğum yapacağım hastaneyi seçme şansım olsaydı, o zaman;
- küvöz başında ROP bakısı yapılıyor mu?
- kontrolleri ve tedaviyi yapan göz doktoru kim?
- uygulanan tedavi yöntemi ne, işlem nerede uygulanıyor?
sorularının cevapları seçim kriterlerim arasında olurdu.

Sonrasında, miyop ve şaşılık beklenmesi gereken olası durumlar. Gözlük takıp, kapatma yapmak gerekiyor genelde, uzun bir süreç bu da. Ama herşey yolunda giderse, bağımsız bir hayat için yeterli görmeyi sağlama şansı var bebeğinizin :)


ROP is one of the nightmares of a lot of preterms.

The risk increases as the birth week gets early and the birth weight is lower. It has "zone" and "stage". It has possible diagnosis and prognosis, but critical control points should not be missed!

In summary the process evolves as follows:
- the vascularization in the retina stops along with birth
- in case of early birth, the vascularization stops before completion
- an abnormal vascularization starts in the retina, considered to be due to high or low oxygen levels with respect to the mother womb
- the area (zone I-II-III) and degree (stage 1-2-3-4-5) is evaluated by ophthalmologists experienced with ROP cases
- stage 1-2 indicate follow-up
- stage 3 indicate intervention, mostly by laser
- stage 4-5 is the period where it is harder to intervene, and vision loss is considered...

If I knew then what I know now and if I had the chance to choose the hospital I was going to deliver in taking into account the possibility of early birth, then;
- is ROP controls made at incubator stand in the NICU?
- who is the ophthalmologist performing the controls and treatment?
- what is the treatment method, where is it performed?
would be questions that the answers would get credit for in my selection criteria.

Afterwards, farsightedness and cross-eye are probabable outcomes to expect. It is necessary to ware glasses and perform eye-patch, it is too a long period. But if all goes well, your baby has a chance to have enough vision to lead an independent life :)
